Buy the book Shenkman and Klein “Living Wills and Health Care Proxies: Assuring Your End of Life Decisions are Respected” from www.amazon.com for more information and samples. Be VERY careful using most inexpensive internet and public forms. Most of them simply don’t address most of the key issues.
