Child Emergency Medical Form.

• What is it.

• If you and your spouse/partner are away on a
  vacation, or business trip (or perhaps a job hunting trip)
  what happens if the kid gets sick?

• Most people write out a note at the kitchen table
  before they leave. That is pretty useless and inadequate.

• Why do you need it.

• You want a limited list of trusted people named and
  authorized to take your child to a doctor and make emergency medical
  decisions.

• You need key data made available for them so that
  they and the doctors can appropriately help.

• What decisions should you make.

• Who should be named as agent.

• What decisions should they make or not make.

• What other steps to take.

• List any key medical issues and information in the
  form.

• If your child has a medical problem, drug use, or
  other issue, disclose it.
